I received this signature from a private signing, and the signature is guaranteed authentic for life. The JSA Witnessed authentication hologram number is WIT241791.
The JSA Witnessed COA is LOST and it is not included. However, due to the cracking on the JSA sticker, they said they would replace the certificate and replace the sticker free of charge. The winning bidder would just have to walk up to them at a car show or a memorabilia show with this jersey and they will replace it, or you could even mail it to them.
